Dear Ms. Davis,

Enclosed is a copy of the U.S. District Court Violation Notice filed against me through the V.A. Police Department. I would like to request your assistance in getting a copy of the police report and the complaintant’s statement. Officer William Lyzenga instructed me to request it through the medical records office who would then request a copy from Bill Cowley, Chief of the V.A. Police. I am unable to make this request at this time and since I believe it is directly related to my complaint concerning The Central Valley Medical Center in Nephi, I think it would be apropos for you to make the request.

I have also enclosed a copy of the restraining order filed against me by "Grace" of the Fountain Green Medical Clinic which restricts me from talking to her and various other staff including Physician Assistant Molly Brotherson. If I have interpreted this document correctly, I am barred from receiving medical care since I am not permitted to speak with my physician about my medical needs.

I have included with the copy of the restraining order an affidavit of my own and a point by point response to both the “Acts Supporting Allegation of Stalking” document and Ms. Kaibetony’s own affidavit. These documents are printed on yellow paper to distinguish them from the restraining order.

I appreciate any assistance you can offer in this matter. I am very concerned that the V.A. would enter into a contract with a private sector company that would harass V.A. patients in this way. I feel the contract between the V.A. and The Central Valley Medical Center should be reconsidered if not rendered void altogether.

The very thought of having to go back to the Fountain Green Clinic and even dealing with Ms. Kaibetony on an administrative level aggravates my dysthimic depression and general anxiety disorder.

I want to consult a physician about these and other medical issues I have but I have effectively been restricted from doing so.
